Dr. Jaremy James - Medical Director - Primary Care
Dr. Jaremy James specializes in Primary Care and is board-certified in Internal Medicine. Dr. James is also a diplomate of the American Board of Obesity Medicine. Dr. James has practiced for 11 years and has been affiliated with multiple hospitals and health systems. Dr. James completed his internship and residency at Plaza Medical Center in Fort Worth (now medical City of Fort Worth).
Dr. James enjoys helping his patients understand the disease process so they can take better care of themselves. Dr. James enjoys spending his free time with his family, traveling, woodworking, and relaxing. Dr. James is excited to be here and be a part of the Bell family.
Dr. Shaadi Khademi, MD, MS, FACOEM – Occupational Health Medical Director
Dr. Shaadi Khademi is a board-certified occupational and environmental medicine physician and the new occupational health medical director at Bell Health & Wellness Center. She previously served as medical director for a large healthcare system, overseeing employee health and an occupational health clinic that served many clients across various industries. She has experience performing a broad scope of occupational health services and her special interests include occupational skin diseases, preventive medicine, and patient education.
Dr. Khademi completed her occupational and environmental medicine residency at the University of Texas at Tyler Health Science Center. She also has a Master of Science in environmental sciences and is a fellow of the American College of Occupational and Environmental Medicine. She is committed to protecting the health and safety of workers and their work environments.
Dr. Khademi relocated from East Texas to be closer to family in DFW and is excited about her role as medical director at Bell. She enjoys spending her time outside of work with her rescue dog, Alvin.
Lily Sokolowski, PT, DPT, OCS – Physical Therapist
Lily is an orthopedic physical therapist who has practiced in the DFW area for 8 years. She graduated with her Doctorate in Physical Therapy from Texas Women's University in 2015. After graduating, she pursued advanced certifications as an Orthopedic Clinical Specialist and in orthopedic manual therapy.
Lily believes in looking beyond the symptoms to get to the root of the problem, and incorporates a variety of interventions in her treatments, such as joint mobilizations, dry needling, and tailored exercises. Recognizing the importance of an integrative approach to patient care, Lily also obtained her Master's in Psychology in 2020 as an adjunct to her clinical PT practice. Lily believes quality patient care is rooted in respecting the dignity of the human person and strives to combine clinical excellence and empathy while serving her patients.
Lily is a born and raised Texan from a family of seven. She enjoys all things outdoors, such as kayaking and hammocks by the lake, traveling, and the occasional well-planned prank. She is excited to join the Premise Health team and serve the Bell community!
Shawna Wilson, BSN, RN – Care Management Registered Nurse
Shawna Wilson is the Care Management Nurse at Bell Health & Wellness Center. She holds a Bachelor’s Degree in Nursing from the University of Texas Arlington and has enjoyed a fulfilling career as a Registered Nurse for 20 years. Shawna previously served as a Nurse Care Manager at Optum where she educated patients on disease management with various medical diagnoses.
She has grown professionally through each and every nursing role she has undertaken, from chronic disease to maternal-childbirth education. Shawna is a firm believer in lifelong learning and the endless possibilities that exist in the nursing profession to improve the health and wellness of the patient population that she serves.
Shawna has lived in the Hurst area for 20 years and has 3 beautiful children - Deyana (23), Kendrix Jr. (20), and Jonathan (7). In her free time, she enjoys gardening, swimming, yoga, dancing, and traveling to a beautiful beach.
Shawna is excited to provide support, education, and awareness around disease management to the Bell community starting April 15th.
Megan Chambers, LCSW – Behavioral Health Counselor
Hello, my name is Megan Chambers, and I am a Licensed Clinical Social Worker. I am a born and raised Texan in Tarrant County who loves the Dallas Cowboys, Texas Rangers, and of course Tex-Mex cuisine! I received my Masters of Social Work at the University of Texas at Arlington and have worked in multiple settings, providing counseling services to adults and children of all ages. I have experience working with people in many mental health areas including but not limited to depression, anxiety, anger management, grief, and developing effective coping skills.
I am looking forward to meeting and working with Bell employees and their families, supporting and helping them achieve their goals!
Shanequiwa Sledge, LPC – Behavioral Health Counselor
Shanequiwa is a Licensed Professional Counselor and certified Trauma Therapist. She has worked in multiple settings, including a family violence shelter, schools with at-risk youth, a behavioral health hospital, and a children’s advocacy center. She has worked on multidisciplinary teams that included law enforcement, child welfare, psychiatrists, nurse practitioners, and school administration.
Shanequiwa was born and raised in Fort Worth, Texas, but has also resided in Austin, Waco and Dallas. She attended Huston-Tillotson University, a small HBCU in Austin, TX, where she majored in Psychology, minored in English, and discovered her love for creative writing. She then earned her Masters in Clinical Mental Health Counseling from the University of Mary Hardin-Baylor.
She is a steadfast lover of the Dallas Cowboys and a proud member of the illustrious Delta Sigma Theta Sorority, Inc. Shanequiwa loves spending time with her family and celebrating the holidays. She looks forward to working with the Bell Health & Wellness Center team and the Bell community toward their continued success.
Daisy Williams, FNP – Family Nurse Practitioner
Daisy Williams is a board certified family nurse practitioner. She obtained her BSN from Texas Christian University in 2005 and MSN from UT Arlington in 2012. Daisy is married to a firefighter and has two kids, Landon and Lexi who are 5 and 2. She enjoys traveling, spending time with family, and attending her local church.
Kimberly Washington, PharmD – Pharmacy Manager
Kimberly is a solution-focused pharmacist, with over 20 years of industry experience. Kimberly is passionate about the health and wellness of her patients. Kimberly is a proud 2008 graduate of Xavier University of Louisiana where she earned her Doctor of Pharmacy degree, and in 2018 completed her Masters in Public Health from Benedictine University.
Kimberly uses a team approach when working with patients and other healthcare professionals and is very passionate about the health and wellness of her patients.
Kimberly strives daily to ensure her patients are educated about their medication and is a pharmacist that loves advocating for patients and being honest and transparent. Kimberly strives to provide the best patient experience with all encounters, and constantly works to ensure that patients adhere to their medications allowing them to live long, healthy and happy lives.
Kimberly is a proud wife and mom of four, from Evanston, Illinois. When she is not spending time with family and friends, she enjoys traveling and performing community service acts with her sorority.
Krystal Torrez – Health Center Manager
Krystal Torrez has been a Registered Nurse since 2009 and received her Bachelor of Science in Nursing from Texas Woman’s University. She has practiced in acute care hospital settings including cardiac PCU, neuro med/surg, and acute inpatient rehabilitation units. She earned her Master of Business Administration in Healthcare Management in 2021 and has been in a Healthcare leadership role since 2016. Krystal is a mom of 4 plus 1 cat. When she is not at work, Krystal can be found attending one of the many activities for her children including soccer, jiu-jitsu, and theatre performances. She is also a big football fan and, being from Louisiana, a huge New Orleans Saints and LSU fan.
